ANY FORM OF DISCREPANCY WILL NOT BE RECOGNIZED THROUGH AN AFFIDAVIT.<\/p>\n<p style=\"text-align: center\">***<\/p>\n<p><strong>REMINDERS:<\/strong><\/p>\n<p>1. Personal appearance of every applicant is a MUST.<\/p>\n<p>2. Personal appearance of either parent for minor applicants is a MUST (preferably mothers).<\/p>\n<p>3. Applicants must bring their printed Application Packet<\/p>\n<p>4. Payment is NON-REFUNDABLE.<\/p>\n<p>5. Each applicant should bring an exact amount of 150 php for courier.<\/p>\n<p>6. POW is a 2-day processing. There will be no extension of the appointment and each transaction non-transferrable. No Show is forfeiture of payment and no reschedule.<\/p>\n<p>7. Requirements must be complete on the day of processing.<\/p>\n<p>8. Please be reminded that if an application is not approved due to discrepancy or missing document, payments are forfeited and there will be no extension of appointment and processing.<\/p>\n<p>9.
