Social media helped generate the buzz and now, there are a lot of people who have placed traveling in their bucket lists. Of course, to meet their travel goals, a valid passport is usually needed. (ADULT)<\/strong><\/h4>\n<p>Contrary to what others say, it really isn\u2019t difficult to understand what the requirements are when applying for a passport:<\/p>\n<p style=\"text-align: center\"><img qualified to use the courtesy lane must have a confirmed appointment slot which you can set <a href=\"https:\/\/www.passport.gov.ph\/\" target=\"_blank\" rel=\"noopener\">here<\/a>.<\/li>\n<li>An accomplished application form which you can find <a href=\"http:\/\/consular.dfa.gov.ph\/images\/2017\/consularforms\/New_Application_Adult01.pdf\" target=\"_blank\" rel=\"noopener\">here<\/a>.<\/li>\n<li>Applicant must personally appear at the DFA office of their choice on the date and time of their appointment.<\/li>\n<li>Applicant must have an <strong>ORIGINAL<\/strong> copy of the Philippine Statistics Authority (PSA) Birth Certificate on Security Paper.<\/li>\n<\/ol>\n<ul>\n<li>For married females using their spouse\u2019s last name, an <strong>ORIGINAL<\/strong> copy of the PSA Authenticated Marriage Contract on Security Paper or Report on Marriage is also needed.<\/li>\n<\/ul>\n<ol start=\"4\">\n<li>Any of the accepted valid IDs rel=\"noopener\">DFA Satellite Offices in Metro Manila<\/a><\/em><\/p>\n<h4><strong>SUPPORTING DOCUMENTS<\/strong><\/h4>\n<p>The four requirements are usually all that\u2019s needed when applying for a passport but there are cases when supporting documents are needed.<\/p>\n<h4><strong>LATE REGISTRATION OF BIRTH CERTIFICATE<\/strong><\/h4>\n<ol>\n<li>If the Birth Certificate was registered 10 years ago, it will be treated as a regular application and no additional supporting documents are needed.<\/li>\n<li>If the Birth Certificate was registered less than 10 years ago, the applicant must submit IDs that <strong>PRE-DATE<\/strong> the late registration.<\/li>\n<li>Should the applicant have no IDs that pre-date the late registration, applicant must produce <strong>CURRENT IDs AND<\/strong> a National Bureau of Investigation (NBI) Clearance.<\/li>\n<\/ol>\n<p style=\"text-align: center\"><img loading=\"lazy\" decoding=\"async\" width=\"536\" height=\"534\" rel=\"noopener\"> DFA Accommodates More Applicants with &#8216;Passport On Wheels&#8217;<\/a><\/em><\/p>\n<ol>\n<li>If the applicant was born <strong>AFTER<\/strong> 1950, they have to file for late registration with the Local Civil Registrar (LCR) or Consular Office that has jurisdiction over their birthplace. Proceed as above.<\/li>\n<li>If the applicant was born <strong>ON OR BEFORE<\/strong> 1950, the applicant must submit <strong>ORIGINAL<\/strong> copies of PSA Authenticated Certificate of No Birth Record <strong>AND<\/strong> Affidavit of <strong>TWO<\/strong> Disinterested Persons attesting to the applicant\u2019s identity.<\/li>\n<\/ol>\n<h4><strong>APPLICANT\u2019S MARRIAGE HAS BEEN DISSOLVED AND WANTS TO REVERT TO HER MAIDEN SURNAME<\/strong><\/h4>\n<ol>\n<li>Submit the<strong> ORIGINAL<\/strong> copy of Annotated PSA Marriage Certificate (MC) or Report of Marriage (ROM) stating that the marriage has been dissolved<\/li>\n<li>If annotated MC or ROM from PSA is not yet available, the applicant must submit a Certified True Copy of the Court Order dissolving the marriage <strong>AND<\/strong> a certificate of Finality from the court.<\/li>\n<\/ol>\n<p><em>Also read: <a href=\"https:\/\/primer.com.ph\/blog\/2017\/09\/05\/dfa-removes-travel-agencies-reserved-slots-for-passport-appointments\/\" target=\"_blank\" rel=\"noopener\">DFA removes travel agencies&#8217; reserved slots for passport appointments<\/a><\/em><\/p>\n<h4><strong>BIRTH CERTIFICATE OR REPORT OF BIRTH HAS LACKING DATA<\/strong><\/h4>\n<ol>\n<li>Submit proof of filing supplemental report or correction from the Local Civil Registrar.<\/li>\n<\/ol>\n<h4><strong>DISCREPANCY OF DATA IN BIRTH CERTIFICATE\/REPORT OF BIRTH AND OTHER DOCUMENTS<\/strong><\/h4>\n<ol>\n<li>If the discrepancy is in the Birth Certificate, the applicant must submit annotated birth certificate authenticated by PSA reflecting the corrected entry. The Department of Foreign Affairs only accepts the following valid IDs:<\/p>\n<ol>\n<li>Social Security System (SSS)\/ Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card<\/li>\n<li>Land Transportation Office (LTO) Driver\u2019s License.
